I am the parent/guardian of the registered player. Permission is hereby granted to the attending physician to proceed with any medical treatment as may be necessary resulting from participation in Suzy Whaley Golf programs. In the event of any serious illness, the need for major surgery, or significant accidental injury, I understand that an attempt will be made by the attending physician to contact me in the most expeditious way possible. If the physician is not able to contact me, the treatment necessary for the best interest of my child may be administered.

In consideration of being allowed to participate in any way in Suzy Whaley Junior Golf Academy programs, events, junior league, camps and activities, the undersigned acknowledges and agrees that:

1. There is a risk of injury from the activities involved in this program, including the remote potential for permanent injury and death, and while particular rules, equipment and personal discipline may reduce the risk of serious injury, risks, by the very nature of the activity, golf carries a risk of physical injury. No matter how careful the student and the teacher are, the risk cannot be eliminated. The risk of injury includes minor injuries such as bruises and more serious injuries such as broken bones, dislocations and muscle pulls. Sports or any activity that involves motion and rotation in a unique environment carries with it a reasonable assumption of risk. In consideration for acceptance of the applicant, and in consideration of the applicant having an opportunity to participate in our programs, those legally responsible for the named enrolling student realize the risk of injury involved in the actions and activities conducted by Suzy Whaley Junior Golf Academy, Suzy Whaley Golf, Suzy Whaley, Inc., SWG Ventures, LLC, PE2Tee Foundation, Inc., PE2Tee, PGA Jr. League, League Golf LLC, Farmington Woods Golf Club, and their respective owners, trustees, sponsoring agencies, sponsors, volunteers, advertisers, board members, employees, agents, subsidiaries, contractors, heirs, successors, assigns, and, if applicable, owners and lessors of the premises used to conduct any event or program (collectively, �Releasees�), and agree that all risk cannot be completely eliminated; and
2. I KNOWINGLY AND FREELY ASSUME ALL RISKS, and assume full responsibility for my participation; and
3. I willingly agree to comply with the customary terms and conditions for participation including compliance with all oral and written instructions. If, however, I observe unusual and/or significant hazard during my present or participation, I will remove myself from participation and bring such to the attention of the nearest official or employee immediately; and
4. I, for myself and on behalf of my heirs, successors, assigns, personal representatives, family and next of kin (collectively �Releasors�), HEREBY RELEASE AND HOLD HARMLESS RELEASEES WITH RESPECT TO ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION ARISING FROM ANY INJURY, ILLNESS, DISABILITY, DEATH OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR ANY OTHER ACTS OR OMISSIONS OF THE RELEASEES.
5. I, for myself and Releasors, hereby agree and indemnify, hold harmless and covenant not to sue Releasees and hereby release same against any costs, damages, liens, expenses (including attorneys� fees), lawsuits, claims, procedures, actions or other liability claimed or imposed upon the person or entities hereby released for any property damage or loss, personal injury of any kind, illness and/or death, whether arising from the negligence of the Releasees or otherwise.
6. The undersigned further expressly agrees that the foregoing waiver and assumptions of risk agreements is intended to be as broad and as inclusive as is permitted by the law of the State of Connecticut and that if any portion is held invalid, it is expressly agreed that the remaining terms and conditions shall, notwithstanding, continue in full legal force and effect. The undersigned further agrees that this Agreement shall be construed under the laws of the State of Connecticut and the venue of any dispute between the parties shall be Hartford, Connecticut.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT AND FULLY UNDERSTAND ITS TERMS. I FURTHER UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND FULLY INTEND THIS AGREEMENT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY OF THE RELEASEES TO THE GREATEST EXTENT ALLOWED BY LAW. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

I hereby authorize enrollment of my child in the programs of Suzy Whaley Golf. I further certify that I, as parent/guardian with legal responsibilities for this participant, do consent and agree to his/her release of all the Releasees as provided above, and for myself and all of the Releasors, I hereby release and agree to indemnify the Releasees from any liabilities incident to my minor child�s involvement and participation in these programs as provided above, EVEN IF ARISING FROM THE RELEASEE�S NEGLIGENCE. I fully understand the terms and conditions of this Agreement as set forth above and expressly agree to be bound by the foregoing terms and conditions.

I, being parent/guardian of the registered player do hereby grant permission throughout the universe to Suzy Whaley Junior Golf Academy, Suzy Whaley, Inc., SWG Ventures, LLC, PE2Tee Foundation, PGA Jr. League, League Golf LLC, and each of their respective affiliates, officers, directors, employees, volunteers, workers, members and agents (�Releasees�) to utilize my child/ward�s likeness, image, voice, and/ or words incidental to any pictures, television, radio, videotapes, recordings, film, the Internet, or any other form or medium now known or hereinafter devised without compensation, payment of royalties, notification, or permission for use in printed brochures, announcements or publicity releases, videos, photo displays and/or internet use. I further understand and irrevocably agree that (1) these materials will become the property of Releasees and will not be returned to me; (2) Releasees may edit, alter, copy, exhibit, publish or distribute photos for purposes of publicizing or promoting Releasees, its programs, or for any other lawful purpose; and (3) I waive any right to inspect or approve Releasees� use of my child/ward�s likeness, image, voice, and/or words.

I hereby acknowledge that I have knowingly and willingly entered my child, who is a minor (the "Child"), into the Angel Park Golf Academy (the "Golf Academy") offered at Angel Park Golf Course (the "Golf Course"). In consideration for allowing my Child to participate in the Golf Academy, I do voluntarily and willingly execute this Release of Liability and Acknowledgment and Acceptance of Dangers, Risks, and Hazards Of Participating in a Golf Academy (the "Release") with the expressed intention of effecting a release of all claims, both in tort and in contract, whether past, present, and future, known or unknown, of every kind, character, and description.

I hereby acknowledge, agree, and stipulate that I have chosen of my own free will to allow my Child to participate in the Golf Academy and realize there are inherent dangers from the sport of golfing and related activities, including but not limited to, the danger from other golfers, the danger of injury from the use of golf equipment, and other dangers of any nature whatsoever. I hereby acknowledge that other minor golfers will be present during the Golf Academy, some of whom may pose a particular risk of Injury or other hazard to other Golf Academy participants due to their young age, lack of skill at golf, or both.

I hereby acknowledge that the Golf Academy is a program designed to teach my Child the fundamentals of the game of golf through, among other methods, practice of the golf swmg with use of golf clubs and balls. I acknowledge and agree that the Golf Academy lessons will begin at the time when the Child receives tutorial instruction from one of the Golf Academy instructors and expressly not upon the Child's arrival at the Golf Course. I agree that J alone am responsible for the Child's safety in transporting and supervising the Child in his or her transportation to the Golf Course tutorial facility.

I hereby acknowledge that this document is to warn me that dangerous conditions, risks, and hazards for minor children do exist at the Golf Course, including the dangers of flying golf clubs and balls that may or may not be in the control of any Golf Course player or the Golf Academy. Further, I acknowledge that although the Golf Course may have a greater knowledge of the Golf Course than me or my Child, it is impracticable and impossible for the Golf Academy to list or to physically show me or my Child each and every possible hazard on the Golf Course. I understand my Child's presence and activities on the Golf Course expose both my Child and my property to dangerous conditions, risks, and hazards, and I hereby state that I expressly assume all such dangers, risks, and hazards.

In consideration for allowing my Child to enter into the Golf Academy, I hereby release and agree to protect, indemnify, and hold harmless the Golf Academy and the Golf Course and their respective heirs, agents, employees and assigns from and against any and all claims, demands, causes of action, and damages, including attorneys' fees, resulting from any accident, incident, or occurrence arising out of, incidental to or in any way resulting from the use of the Golf Course and all improvements therein, whether or not caused by the Golf Academy, the Golf Course, their respective agents, representatives, employees, successors or assigns, or the negligence of any of the foregoing. I hereby further consent and agree that I and my heirs, successors, and assigns will not make any claim or institute any suit or acton at law or in equity against the Golf Academy, the Golf Course, or any of their respective heirs, agents, representatives, employees, successors, or assigns by reason of conditions of the Golf Course or activities occurring thereon.

I agree to extend this release to include any golf outings, trips for practice rounds, or tournaments my child participates in with any instructor of The Summerlin Golf Academy away from Angel Park Golf Course. This release would begin when I release my child to the instructor and end when my child is returned to me.

I HAVE READ THIS RELEASE OF LIABILITY AND ACKNOWlEDGEMENT AND ACCEPT THE DANGERS, RISKS AND HAZARDS OF PARTICIPATING IN A GOLF ACADEMY AND UNDERSTAND ALL ITS TERMS. I EXECUTE IT VOLUNTARILY WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
